	Initial Monitoring Well	$356.00per parcel	not included in site mitigation fee
	Subsequent Monitoring Well	$89.00 each	if on same site
	Vapor Extraction Well	$356.00per site	included in site mitigation fee
	Well Abandonment	$178.00	per well or monitoring well
Backfilling Requirements for Borings	Cement - tremie tool must be used for cement seal if > 20'.
Backfilling Requirements for Monitoring Wells	Cement/Bentonite. Notify inspector so that he may be on-site while being tremie grouted
Inspection Fees	Notify inspector so that he may be on-site
Permit Application Information	Application required to verify proper license.
