	Subsequent Monitoring Well	$50.00	If inspected w/1st well
	Subsequent Monitoring Well
	Groundwater Sample	None
	Groundwater Sample
	Vapor Extraction Well	None (if soil only)	L.O.P. Site
	Well Abandonment	$75.00/hr	Unless site is an L.O.P. case.
Backfilling Requirements for Borings	Generally needs to be filled with neat cement/ sand cement grout. See UT-50
Backfilling Requirements for Monitoring Wells	Same As Above, With 3-5' Bentonite Seal.
Inspection Fees	yes - inspection required. Please provide 24 hrs notice. $75.00/hr if re-inspected.
Permit Application Information	Permit can be processed immediately if you call and make an appointment. Normal processing is required if no appointment is made. UT-50 Hand Book.
