Permit Information

Permit Fees	Boring	no fee	Permit required for borings greater than 45 feet.
	CPT Boring for lithologic logging	no fee	Permit required for borings greater than 45 feet.
	Initial Monitoring Well	no fee	1 permit per casing
	Subsequent Monitoring Well	no fee	1 permit per casing
	Groundwater Sample	no fee	E-Boring permit required if greater than 45 feet (one cluster permit required).
	Vapor Extraction Well	no fee	1 permit per casing
	Well Destruction	no fee	1 permit per casing Backfill with cement grout or neat cement only.
Backfilling Requirements for Borings	Neat cement, volclay, enviroplug in slurry form only. 10^sk, cement-sand grout slurry. No bentonite pellets.
Backfilling Requirements for Monitoring Wells	Neat cement with no more than 3 ft of bentonite pellets in transition zone, hydrated volclay, enviroplug in slurry form only. 10^sk, cement-sand grout slurry. 2" space between multiple casing.
